2/23/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her morning. She verbalized to QMHP-C that she was having a good morning and that she was up early this morning. QMHP-C informed Ashiyah that this session was going to focus on a family session to discuss their level of communication. 2. Behavior: Ashiyah contributed to the session and did not need any redirection throughout the session as she participated fully in session. Ashiyah was able to discuss with QMHP-C additional ways she can learn how to communicate with her mother such as through the use of text message or through the use of writing in on paper. 1. Intervention: QMHP-C began session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C reviewed with Ashiyah and her mother in healthy communication skills and expressing feelings and thoughts appropriately. QMHP-C educated Ashiyah on being more assertive as she has difficulty with verbalizing her thoughts and active listening skills. QMHP-C actively listened as Ashiyah expressed her feelings regarding her motherâs report of her noncompliant and disrespectful behavior in the home this week. 2. Intervention: QMHP-C encouraged Ashiyah to stop and think before she reacts, especially when her mother makes her angry, and to utilize learned coping skills. QMHP-C reminded Ashiyah that she is accountable for her actions and behavior. QMHP-C educated Ashiyah on appropriate ways to communicate with authority figures by showing respect, and learning to verbalize her feelings instead of acting out feelings. QMHP-C modeled and engaged the Ashiyah in an exercise to support her with communicating and understanding age appropriate communication and interactions. 1. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. In the beginning of session Ashiyah was in a positive mood. However, when she was confronted about her behaviors this week (as reported by her mother) she felt that she was being attacked for her behaviors. QMHP-C had to reiterate that communicating thought her actions and feelings was to support her with identifying coping skills that would improve her communication skills with her mother. 2. Response: Ms. Hodge reported that Ashiyah had been acting inappropriately all week and Ashiyah refused to communicate any problematic behaviors. Ashiyah reported that she had been upset with her mother because she does not allow her to go to her friendâs house and that is not fair to her because she wants to spend time with her before school starts back because she knows that she is not going to allow her to go across the waters.
